1例犬疑似广州管圆线虫感染的病例报告

摘要: 犬神经性血管线虫病(CNA)是由广州管圆线虫感染引起的一种以严重的神经功能障碍为主要症状的寄生虫病。国内尚未有关于广州管圆线虫感染病例的报道。该病例是1头5岁的雄性比熊犬,疑似因误食非洲大蜗牛后出现后肢瘫痪来就诊。经临床检查、血液学检查、脑脊液检查、核磁共振成像(MRI)和PCR检查后,症状符合广州管圆线虫感染,但因患犬就诊三天后死亡,未进行治疗,且未进行CSFPCR检测,故高度怀疑该病例为国内首例广州管圆线虫感染。该文通过对该病例的分析讨论,从临床症状,检测方法以及防控管理进行探究,以期为相关的临床诊疗提供参考。

Abstract: Canine neurla angiostrongylosis (CNA) is a parasitic disease caused by the infection with Angiostrongyliasis cantonensis,characterized primarily by severe neurological dysfunction. This article discusses the diagnosis,prevention and control of CNA,aiming to provide a reference for clinical practice. A 5⁃year⁃old male Bichon Frise was presented with paralysis after reportedly ingesting a giant African land snail. Diagnostic investigations included clinical examination,complete blood count,cerebrospinal fluid(CSF)analysis,magnetic resonance imaging(MRI),and polymerase chain reaction(PCR)testing. CSF smear examination revealed elongated structures morphologically consistent with parasitic nematodes. MRI findings included T2 ⁃ weighted hyperintense signals in the thoracolumbar spinal cord and mild leptomeningeal enhancement in the brain. Based on these findings, the history of possible snail ingestion,and the endemic geographical location,a definitive diagnosis of A. cantonensis infection was confirmed. The dog died three days after presentation without receiving treatment. This case indicates that canine neural angiostrongylosis is rapidly progressive and carries a high mortality risk,underscoring the need for prompt clinical recognition and effective diagnostic and therapeutic interventions.
